Feedback on Accessibility
The Public Health Agency of Canada (PHAC) has developed an Accessibility Plan, as set out by the Accessible Canada Act, which came into force in June 2019. This Plan aims to eliminate accessibility barriers for Canadians served by the Agency. The Plan is available here on this webpage, and will be updated every three years per the requirements of the Act.
We would like to invite you to share your views on:
- accessibility barriers that exist at PHAC;
- suggestions on how we can break down barriers for clients and Canadians and make PHAC a more inclusive workplace;
- accessibility best practices;
- success stories related to accessibility;
"Barrier" means anything (including physical, architectural, technological or attitudinal) that is based on information and communications, or anything that is the result of a policy or practice that hinders the full and equal participation in society of persons with a physical, mental, intellectual, learning, communication or sensory impairment or a functional limitation.

The personal information you provide to Health Canada and the Public Health Agency of Canada will be used by the Accessibility Readiness Team (ART), under the authority of the Accessible Canada Act and handled in accordance with the Privacy Act.
Why are we collecting your personal information?
Your view, opinions, and contact information, if you chose to provide it, will provide insight into barriers to accessibility within Health Canada and the Public Health Agency of Canada, and would allow us to follow up with you regarding this feedback form. It is possible the responses you provide could be used alone, or in combination with other available information to identify you. The protection of your personal information is very important to us and we will make every effort to safeguard it and reduce the risk that you are identified.
Will we use or share your personal information for any other reason?
We will use your personal information to help us address any barriers and inform policy development. If you provide your name and email address, ART may contact you for future communications and/or engagement activities.
What happens if you do not want to provide your personal information?
Completion of this form is voluntary; there is no obligation to include your name and email address.
What are your rights?
You have the right to access and request a correction and/or notation to your personal information. You also have a right to complain to the Privacy Commissioner of Canada if you feel your personal information has been handled improperly.
